Program Specific Application Information

Each candidate must be a graduate of an ACPE- accredited College of Pharmacy and eligible for licensure in the state of Wisconsin.

All applicants must use the Pharmacy Online Residency Centralized Application Service (PhORCAS) system and participate in the National Match.

Applicants Must Submit

• Application (via PhORCAS)
• Curriculum vitae
• College of pharmacy transcripts
• Three letters of recommendation
• Letter of intent
• Copies of two projects/presentations using the supplemental attachments in PhORCAS
