Hospice Referral Resources in Wisconsin
SSM Health at Home’s hospice team consists of the patient’s physician, hospice nurses, hospice medical director, medical social workers, spiritual and grief counselors, home health aides, and volunteers.

Hospice Program Services
Our hospice program provides compassionate care to both patients and their families. In addition to our care team, our hospice program can also provide:
- Music therapy
- We Honor Veterans program
- Bereavement services
- Grief support groups
- Camp GLOW (grief support day camp for children in Dane and Sauk Counties)
- SSM Health Hospice House (located in Baraboo, WI)
- SSM Health Hospice Home of Hope (located in Fond du lac, WI)
Hospice Service Requirements
A referral to hospice may be made by the patient, family member, physician, clergy, or other agency. After referral SSM Health at Home will contact the patient’s physician to see if hospice care is appropriate. We will then contact the patient and family to explain the program and to see if they are interested. Hospice admission can then occur.
Patients do not need to homebound to receive hospice services. Care can be provided in the patient’s home, the home of a family member, assisted living facility, nursing home, or residential hospice facility like the SSM Health Hospice House.
The Medicare Hospice Benefit covers the services provided by the hospice team, as well approved medications related to the terminal illness. Many private insurance companies also provide coverage for hospice services. We accept Medicare/Medicaid and a variety of additional insurance plans. Our staff can assist in determining coverage.
