Goals and Objectives:
- Provide safe, efficient and high-value patient centered care, with goals of continuous improvement
- Embrace the culture of safety in daily work
Learn PSQI concepts:
- Introduction to QI concepts including LEAN and PDCA methodologies and A3 problem solving
- Introduction to Patient Safety concepts and collaboration with our Quality Safety Specialists
- Utilization of SSM Event Reporting System
- Identification, recognition and reporting of adverse events & medical errors with confidence
- Participation in root cause analysis
- Advanced training in faculty-supervised transitions of care (hand-offs)
- Active participation in our Transitional Care Clinic
- Focus on QI hospital-based metrics including readmissions
- Elaboration of individual/group QI projects each academic year in collaboration with Academic Biostatistician
- Application of LEAN and PDCA methodologies in individual/group QI projects
- Patient Safety-associated case presentation at Morbidity and Mortality Conferences
- Participation in the ACP MO chapter QI poster presentation
Internal Medicine Performance Board
Recurrent Performance Board meeting:
- Conducted by faculty, driven by residents
- Main forum to address and discuss problems through the residents’ eyes (includes analysis, gap identification and solutions)
- Focus on adherence to the ACGME 80-hr work rule
- Feedback on resident work flow including adherence to ACGME (Accreditation Council for Graduate Medical Information) common program requirements
- Focus of adherence to CLER-related recommendations and expectations
IM Clinic Performance Board meeting:
- Multidisciplinary discussions on several ambulatory care metrics
- Focus on chronic disease-related practice metrics
- Focus on population-health and healthcare disparities
- Focus on patient care quality improvement strategies in our medical home
