SSM Health at Home offers several specialty services and programs to meet the unique needs of our patients.
Orthopedic Specialty Care
SSM Health at Home can support your recovery after orthopedic surgery. We work closely with your physician to coordinate an individualized treatment plan focused on the highest quality outcome – and your satisfaction and comfort.
Orthopedic care is available after:
- Total joint replacements
- Fractures
- Spine injuries
- Orthopedic surgical repairs
Our staff is trained in the latest orthopedic techniques and protocols. Patients benefit from the proven expertise of our experienced team of clinicians which includes registered nurses, physical therapists, occupational therapists, and social workers. The team coordinates with your physician to ensure care is provided according to your specific needs.
Parkinson’s Program
SSM Health at Home provides unique and specialized care for patients with Parkinson's disease, a progressive disorder of the nervous system. This disease affects movement, sometimes starting with a barely noticeable tremor in just one hand and gradually progressing to stiffness or slowing of movements. Being able to move freely and easily is crucial to being able to safely perform daily routines. Our professionals can help!
Our staff is comprised of licensed clinicians including registered nurses, physical therapists, occupational therapists, and speech therapists. These clinicians have the skill sets needed to address your specific needs, whether that be medication management, assisting with daily activities, improving strength and movement, or addressing speech and swallowing problems.
Lee Silverman Voice Treatment (LSVT)
SSM Health at Home therapists are certified in LSVT, which is an evidence-based program that:
- Focuses on physical exercises to improve ambulation and activities of daily living.
- Provides home health services four consecutive days per week for four weeks.
- Has been proven to enhance speed (legs and arms), balance, and quality of life.
Wound Care
The Wound Care Certified® (WCC) nurses at SSM Health at Home are available for consultation regarding our wound care patients. All home care clinicians are trained in simple and complex wound care and treatment. We also utilize specialized software to monitor wounds and communicate changes with the home health team and the patient’s physician.
Common wound treated include:
- Venous leg ulcers
- Pressure injuries
- Surgical wounds
- Diabetic wounds
When a patient with a wound is admitted, a note is entered in the medical record, and the wound care team is notified. Home health clinicians monitor the wound during visits and reach out to the wound care team with questions about treatment or if the wound is not healing as anticipated.
Our wound advisor software allows the wound care team to see and track wounds via the patient’s visit notes. Home health clinicians can upload photographs of wounds, measurements, and other information into the software during their visits. This allows the wound care team to monitor patient wounds, communicate changes, and share information with the patient’s physician.
