Our donors have helped make SSM Health at Home one of the leading in-home health care and hospice providers in southern Wisconsin. Through your donations you are touching the lives of thousands – whether they are patients facing a surgical recovery or chronic illness, families facing a devastating disease or end of life, children and adults grieving the loss of a loved one, seniors who risk going hungry, or people in need of equipment or supplies to maintain their independence.

Your Gift, Your Choice

You can choose to direct your donation to any of the following funds or make an unrestricted gift that may be used by the foundation for the area of greatest need.

Your donation to our area of greatest need supports the integrated and comprehensive variety of programs SSM Health at Home provides including: home health services, hospice, home medical equipment, and community health services.
Your donation supports our volunteer and grief support programs (both of which are required by Medicare/Medicaid, but are not reimbursed), upgrades and maintenance of the SSM Health Hospice House residential hospice facility, and capital needs of the hospice program.
While most people would prefer to stay at home during their final days, sometimes that is not possible. Your donation helps to cover room and board costs at the SSM Health Hospice House for those who are unable to afford them. While Medicare, Medicaid, and most private insurance covers care associated with hospice services, they do not cover room and board costs for residential facilities. Your donation ensures that anyone, regardless of their financial situation, has access to the peaceful, home-like setting provided by the SSM Health Hospice House.
Your donation helps provide our hospice patients with a wish as they near the end-of-life. Past wishes include: purchasing equipment for a bed-bound patient to take a bath; a limousine ride to enjoy a favorite meal; creating scrapbooks to preserve memories; a wedding ceremony; and a fishing trip to a local lake.
SSM Health at Home's Meals On Wheels volunteers deliver hot, nutritious meals, Monday - Friday, to individuals in need in Madison, Middleton, Monona, and Sun Prairie. Your donations support the program by providing funds for those who may not have the means to pay for meals, ensuring those in need do not go hungry.
Camp GLOW is a grief support day camp for kids who have been affected by a loss. Since children and teens often experience and express grief in different ways than adults, the camp is designed to give children a safe place to share their feelings and concerns with a wide range of activities and enough counselors on-site to give individual attention if needed. The camp is offered at no charge to the participants.
